Welcome to our insurance information page. The following links are provided to assist you in gaining all the needed information to determine your benefits for counseling. By contacting your insurance provider, before you come for a session, you will be able to pre-certify your counseling sessions, determine your co-pay (Specialty), and learn if you have a deductable and if you need an authorization number. If you have any problems with your pre-certification, please let us know.
If your insurance company is not listed, then contact them and determine your out-of-network benefits. In Arkansas, we are an “any willing provider” state and benefits are available even if our Counselors are considered out-of-network. Daily Bread Counselors will be able to work with you to determine your payment.
Remember, the client is ultimately responsible for payment.
